Community Health Needs Assessment

A Community Health Assessment is defined as a local health assessment that identifies key health needs and issues through comprehensive, systematic data collection and analysis. Our goal with this assessment is to identify the needs and resources of our community and develop strategies to address Bell County’s health needs and identified issues. 

When the assessment is complete, a Community Health Improvement Plan (CHIP) will be created in order to plan for targeting the opportunities for improvement that were identified and leveraging the communities’ assets.

When the final draft of the Community Health Assessment is ready, the information will be posted on our website and our Facebook page.

We are expected to have the final draft by December 31, 2023.
